NEW! Person-Centered Dementia Care Planning

Dementia is rarely treated as the terminal disease that it is. Standard advanced directives do not address the complex and unique issues that arise with advanced dementia, and more importantly, how one dies when living with dementia. Proactive planning is essential after a diagnosis of dementia. This workshop will share valuable resources on advance care planning for people with dementia so you can get the care you want and avoid the care you do not want. We'll discuss the Dementia Values and Priorities Tool which is designed to help you communicate your wishes regarding future care if you are living with dementia. After answering a series of questions related to the changes commonly seen in the progression of dementia, the tool will provide you with a document that can be added to your existing advance directive and shared with others.

Retired Clinical Psychologist. Over the course of her career, Dr. Pergament designed, implemented, and managed comprehensive psychological intervention programs for individuals and families. In her time as a psychologist with Allina Health, she worked with multigenerational families, helping aging people manage chronic and terminal illnesses, providing education on self-advocacy, and navigating the healthcare system.
